How to Register

Registering for the 2012/13 Summer Competition is easy.

You MUST have an FFA Number!!
Go to www.myfootballclub.com.au to either find your FFA number or create a New FFA Number!
1. Each Team completes a Team Nomination Form!
2. Each Player completes a Player Registration Form!
3. Then just send us the forms and the relevant fees!

You can type your information straight from your pc
onto the forms then just click send

Make sure you also attach a photo and ID if required

You can register by Post, Email or at one of our Registration Days.


Please note if you were not registered with the CDSFA during the winter season, you must register in person at one of the Registration Days, provide proof of identification and have a digital photo taken by one of our staff.


Registration Days

Registering by Post or Email

Register your team or individually by attending one of our Registration Days at Lees Park Ashbury (Canterbury Clubhouse) on one of the following dates:

 

• Saturday 13th October 9am - 11:30am
• Thursday 18th October 66pm-9pm
     

Players registered with the CDSFA in 2012 or who played Summer Football at CJFC last year, are not required to provide proof of age/identity. New players to the CDSFA for the 2012/13 Summer Competition must provide one of the following:

  • Passport
  • Birth Certificate
  • Drivers License

Also, a Digital Photo will be taken by one of our staff.

Your Team Manager can register your entire team provided all players participated in Summer Football at CJFC last season or played Winter Football with CJFC during 2012 (All Player Registration Forms must be fully completed and signed)

  1. Nominate a Team Manager who will download the Team Nomination Form
  2. Each player must download and complete a Player Registration Form
  3. Team Manager must send the completed Team Nomination Form either by post or email together with a $200 fee which will secure your teams spot and kits
  4. Each players must send their Player Registration Form by post or email together with the relevant Registration Fee and photo if applicable (no photo required if registered with CJFC during 2012 or summer football at CJFC last season)
  5. Once at least 8 players are registered and paid for, your team will be officially entered into the competition

An easy way to register your team fast is have your Team Manager post or email the Team Nomination Form, all the Player Registration Forms and relevant fees for the entire team.

Players registered with the CDSFA in 2012 or who played Summer Football at CJFC last year, are not required to provide proof of age/identity.

New players to the CDSFA in 2012/13 Summer Competition must register in person at one of the Registration Days and provide a birth certificate, driver license or passport and have a digital photo taken.

Junior Team Coaches/Managers must provide a signed 'Prohibited Employment Declaration'. Click here to complete the delclaration.

We recommend the Team Manager send in your ‘Team Nomination Form’ first so that your spot is secure and then all you need to do is make sure you have 8 players registered by 9th October 2011

How to Lodge Forms

Return Team Nomination Form, Player Registration Form and Cheques to:

In Person: Lodge Forms at one of the registration days
Email: Fill out the form online and click send. Your email application will open with the form attached. Then send to info@6aside.com.au (include digital photo if required)

Fees Table

Age Groups/Divisions

A player’s age group is determined by their age on 31st December 2011.

Mixed Under 6 - Mixed Under 9
Teams can be comprised of boys, girls or any combination thereof

Under 10 - Under 16
Boy’s competitions are restricted to boys only
Girl’s competitions are restricted to girls only

Men’s Open Age Divisions
All age Mens competition.
Team must state their level on the Team Nomination Form

Men’s Over 35 Divisions
Men must be Over 35 to play in this division
Team’s allowed one (1) player under 35yr
Team must state their level on the Team Nomination Form

Ladies’ Open Age Divisions
All age Womens competition.
Team must state their level on the Team Nomination Form

Men and Women Mixed Division
All age Men and Womens competition
Team must state their team level on the Team Nomination Form

Team Ability Level

Please select your Team's ability level accurately and honestly as it will ensure all competitions are fair, even and as competitive as possible.

To select your team level just choose a level from 1-10 on the Team Nomination Form.

For example, choose level 1 if you have 2 or more players who have played professionally here or overseas or level 10 if your team is made up of players who are playing football for the 1st time. If your team is somewhere in between then select the level which you feel best indicates your team's overall ability.

Be honest with your team level as your team may be moved up or down a division if it is considered to strong or too weak for its current division. This will disadvantage your team as your team will be placed in equal last position if moved to a different division.

Our competition is for EVERYONE of ALL AGES and ABILITIES.
